A common error in incident investigations is starting off with the idea that “the performer is at fault.” This leads to confirmation bias questioning in investigation techniques – and ineffective solutions. This session will illustrate the importance of applying a three-step process (the trilogy) for the review of safety events and incidents. You’ll gain unique insight from the perspective of safety, behavioral science and human factors, as well as organizational practices and culture. You’ll understand the importance of incorporating the trilogy process and its ability to help lead you to the creation of effective remedial strategies to prevent a recurrence. The trilogy process is designed to help guard against the traps often seen in traditional incident investigations. It’s applicable in nearly all types of industries and operations in both the public and private sectors.
